At Sunshine Specialty Health Outreach Foundation, our case management services are designed to help individuals take the next step toward care, stability, and better health. We understand that navigating healthcare, medical referrals, community programs, and social support services can feel overwhelming, especially when someone is facing a new diagnosis, financial hardship, limited transportation, lack of insurance, or uncertainty about where to begin.

Our team provides compassionate guidance and resource connection to help individuals access the support they need. Through case management, SSHOF helps connect clients to medical care, behavioral health resources, HIV and Hepatitis support, STI education, testing resources, social services, and trusted community partners throughout Orlando and Central Florida.
